Lessons Learned from a Multiple Casualty Blast Incident

Introduction: On February 24, 2021, a large explosion and fire occurred in an industrial estate Singapore. Eight casualties with major burns were conveyed to the emergency department (ED) at Singapore General Hospital, designated regional Burns Centre. This article details events recommendations arising from this multiple casualty incident. Method: An After-Action Review (AAR) was conducted examine prehospital notification process, triage & tagging, medical management, manpower resource allocation, command-and-control structure. Results: All eight by national Emergency Medical Services (EMS) arrived within 46-minute window. The first three suffered 90% full-thickness inhalational injuries intubated. remaining five between 37% 64% burns, requiring intubations as well. Four areas identified for improvement: Firstly, there scant information EMS regarding total count. There also inadequate knowledge of mass protocols ED. Thus, resources heavily utilized - all whom later deemed unsalvageable, given palliative care demised. Secondly, identities initially unknown. They tagged similar-looking ten-digit serial numbers, resulting near-miss event involving mislabelled blood tubes. Thirdly, unfamiliarity incident response plan scale. contributed lack situational oversight inconsistent leadership direction various stakeholders conflicting instructions. Fourthly, routine trauma computed tomography pan-scans caused delayed reporting Radiology created bottleneck disposition. A multi-disciplinary workgroup comprising Emergency, Trauma, Intensive Care departments outlined several based on AAR findings. Drills strongly recommended resume following hiatus due COVID-19. Conclusion: AARs help provide invaluable insights. Response plans should be refined together relevant stakeholders.
